I am usually available weekdays between 8.00 and 6.00, with some flexibility.

Payment of Services

Full payment is requested at the time of service. Payment may be in the form of cash or check.

If your treatment is covered by your health insurance, I will bill your insurance company.

I am an in-network provider for:

Kaiser Permanente, Blue Cross Blue Shield (local and federal), Cigna, ODS, Aetna, Providence, CHP, Lifewise, United Healthcare, and Health Net.

If you should need to cancel or reschedule an appointment, please do so at least 24 hours before the appointment time, or you will be charged a $25.00 cancellation fee.

Our Location

The clinic is located opposite New Seasons on Scholls Ferry Rd in Raleigh Hills, close to the intersection with Beaverton Hillsdale Highway.
